Few people in the US would have noticed the link with the actual place. Nowadays, though, too many people in France have become familiar with what I do, and they would immediately pick up on the "ruse." I'm sure they'd have a good laugh! Someone once suggested I draw the map of France in the Mystara style, imagining France as if it were a D&D kingdom. I might yet do this someday. More so a surprise and amusement to see the not too exciting industrial city (the motto "Sundsvall in our hearts" was improved by witty inhabitants with "...and in our lungs") elevated to imperial magic wonderful megapolis status. Back in the 80's when I used these names, I had imagined early Alphatia had been partly settled by folk from the Northern Reaches (Mystara's equivalent to real world Vikings). Nowadays, of course, since this stuff is going over the internet, a lot more people discover these bogus names and react in different ways. I sort of got the gist of this article using Google Translate -- a very poor translation, but enough to get the idea. Hope you enjoy the fantasy version of "Sundsvall" nonetheless!
